Position Overview
We are seeking a passionate and skilled Licensed Mental Health Professional (LMHP) with experience treating Substance Use Disorders (SUD) to join our Assertive Community Treatment (ACT) team.
This is a highly community-based position, with approximately 80% of services provided in clients’ homes and other community settings. The LMHP will work as part of an interdisciplinary ACT team to deliver integrated mental health and substance use treatment to adults with serious mental illness, helping individuals achieve recovery, stability, and greater independence in their daily lives.
Primary Responsibilities
• Provide individual and group substance use treatment using evidence-based practices.
• Provide individual and group substance use treatment using evidence-based practices.
• Conduct comprehensive clinical and substance use assessments.
• Develop and implement individualized treatment plans addressing both mental health and substance use needs.
• Deliver intensive ACT services primarily in client homes and community settings.
• Participate in daily ACT team meetings and collaborate with psychiatric, nursing, vocational, and peer support staff.
• Assist clients in accessing community resources, building natural supports, and achieving recovery goals.
• Complete accurate and timely clinical documentation in accordance with agency, regulatory, and payer requirements.
General Clinical Responsibilities
• Provide rehabilitation, skills restoration, care coordination, and recovery-oriented support services within the scope of licensure and expertise.
• Participate in treatment team meetings and coordinate care with other providers as needed.
• Document all client contacts and services provided.
• Maintain knowledge of DMAS, DBHDS, and Managed Care Organization (MCO) requirements and ensure compliance with all applicable regulations.
• Maintain current credentials and required personnel documentation and notify Human Resources of any changes affecting employment status or job duties.
Qualifications
• Current LMHP licensure in in Virginia: LPC, LCSW, LMFT, LCSW.
• Licensed Mental Health Professional - Resident
• Licensed Clinical Social Worker- Supervisee
• Experience providing substance use disorder treatment.
• Ability to work independently in community settings and travel throughout the service area.
• Strong clinical assessment, treatment planning, and documentation skills.
• Commitment to person-centered, recovery-oriented care.

Compensation: $65,000.00 - $70,000.00 per year
