Responsibilities:
- Provide direct patient care, including administering medications, treatments, and procedures as ordered by physicians or advanced practice providers
- Monitor and assess patient conditions, documenting observations and reporting any changes to the healthcare team
- Collaborate with other healthcare professionals to develop and implement patient care plans
- Educate patients and their families on healthcare topics, including disease management, medication administration, and self-care techniques
- Maintain accurate and up-to-date medical records using electronic health record systems such as Epic
- Ensure compliance with infection control protocols and maintain a clean and safe patient environment
Requirements:
- Valid LPN license in Virginia
- Three (3) years of experience working with adults who are diagnosed with a serious mental illness.
- Excellent communication skills to interact with patients, families, and healthcare team members
- Must be able to travel to clients' homes in the community as needed.
